HarbourFront Health Group produced and negotiated a 5-year Business & Operational Plan for the Ottawa Westend FamilyCare Clinic - Family Health Team (FHT), involving multi-stakeholders. This ambitious project received approval of capital and operating grants from the Ontario Ministry of Health & Long-Term Care (MOHLTC).

A needs and demographic analysis was conducted to develop optimal programs to care for 15,000 clients in the region. This plan also included HHR models for Chronic Disease Management (CDM) Programs for diabetes, hypertension and mental health. A full facilities space and equipment analysis was conducted in alignment with the project Recruiting Plan for healthcare professionals and program development.

On request of Oneida Nation of the Thames First Nations peoples and Health Canada (FNIHB), for development of culturally sensitive, primary care services, HFHG is conducting multi-stakeholders dialogue to develop a proposal to improve regional services. In collaboration with numerous stakeholders we have developed a full environmental scan of resources, needs analysis and a framework for regional strategic service integration to ensure a continuum of care for the South Western Ontario Region. This project has gained the support of the federal Aboriginal Health Transition Fund (AHTF) as part of its integration planning process.
